At times, it may be necessary for them to make personal enquiries, on behalf of the doctors. This is necessary to guide the doctors as to the nature and urgency of a medical problem. Please be assured, the Care Navigators are bound by strict rules of confidentiality. They have a difficult job, so please be patient and help them to help you.

Mark Mackrell attends the practice to run a Carers Health clinic, in this clinic, they would look to provide unpaid carers with information and guidance along with practical and emotional support, so they feel valued and supported. They want to help solve financial concerns or alleviate the often-felt feelings of isolation and loneliness, these appointments can be booked via our PCA team.

Social Prescribers help to reduce health inequalities by supporting people to unpick complex issues affecting their wellbeing.
